ELM Group – Long term conditions
At ELM Group we have adopted an innovative approach to the way we care for patients with long term conditions (asthma, diabetes, stroke, hypertension, COPD, coronary heart disease).
The practice has over 3000 patients with one or more of these conditions and we have a responsibility to review everyone at least once a year. It is important to check that everything is going well with their management and any medication they are using. Reviewing patients in this way represents about 30% of the entire workload of the practice.
Our Long Term Medical Condition Clinics (LTC) are run by our specialist nurses who have developed expertise in these areas. By addressing all of the conditions together in one single visit we aim to provide a more holistic approach to patient care. A combined visit also reduces the need for patients to make multiple visits to the practice to see the nurse or doctor. By carrying out these reviews more efficiently we hope to achieve the following objectives

|•||Reduce the need for patients to make multiple visits to the practice to see the nurse or doctor|
|•||Provide a more “joined-up” approach to the care of patients with more than one LTC|
|•||Better allocation of the practice resources so that we have more time for providing good quality health advice for all of our patients.|
